<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Bước 1: Doanh nghiệp đề nghị cấp giấy chứng nhận đủ điều kiện kinh doanh hoạt động thể thao gửi hồ sơ đến Trung tâm Phục vụ hành chính công tỉnh.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Địa chỉ: 70 Lê Hồng Phong, thành phố Kon Tum.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Thời gian: Vào giờ hành chính từ thứ 2 đến thứ 6 hàng tuần.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Bước 2: Cán bộ tiếp nhận kiểm tra tính hợp lý và đầy đủ của hồ sơ:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Trường hợp hồ sơ đầy đủ thì công chức viết giấy hẹn </span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Trường hợp hồ sơ còn thiếu và không hợp lệ thì công chức hướng dẫn về chỉnh sửa, bổ sung.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Trường hợp hồ sơ cần sửa đổi, bổ sung Sở Văn hóa, Thể thao và Du lịch thông báo trực tiếp hoặc bằng văn bản những nội dung cần sửa đổi, bổ sung đến doanh nghiệp trong thời hạn 03 ngày làm việc, kể từ ngày nhận hồ sơ.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Trong thời hạn bảy ngày làm việc, kể từ ngày nhận đủ hồ sơ hợp lệ, Sở Văn hóa, Thể thao và Du lịch phải kiểm tra các điều kiện kinh doanh hoạt động thể thao của doanh nghiệp theo quy định và cấp giấy chứng nhận đủ điều kiện kinh doanh hoạt động thể thao.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Trường hợp không cấp, Sở Văn hóa, Thể thao và Du lịch trả lời bằng văn bản và nêu rõ lý do.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Bước 3: Trả kết quả tại Trung tâm Phục vụ hành chính công tỉnh hoặc qua đường bưu điện.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Thời gian: Vào giờ hành chính từ thứ 2 đến thứ 6 hàng tuần.</span></span></div>

<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Bản sao Giấy chứng nhận đăng ký doanh nghiệp; bản sao văn bằng, chứng chỉ, giấy chứng nhận của nhân viên chuyên môn đối với cơ sở kinh doanh hoạt động thể thao thuộc Danh mục hoạt động thể thao bắt buộc có hướng dẫn tập luyện hoặc cơ sở kinh doanh hoạt động thể thao mạo hiểm.</span></span></div>

<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">1. Sân bóng rổ</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">a) Sân bóng rổ có chiều dài ít nhất là 28m và chiều rộng ít nhất là 15m;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">b) Mặt sân bóng rổ phẳng, cứng, không trơn trượt và không có chướng ngại vật;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">c) Đối với sân bóng rổ ngoài trời, hàng rào, lưới chắn hoặc tường bao quanh sân cách đường biên dọc của sân ít nhất là 2m và cách đường biên ngang của sân ít nhất là 5m. Trường hợp sân bóng rổ ngoài trời liền kề nhà ở, trường học, công trình công cộng hoặc đường giao thông, hàng rào, lưới chắn hoặc tường bao quanh sân bóng rổ cao ít nhất là 3m;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">d) Đối với sân bóng rổ trong nhà, chiều cao tính từ mặt sân đến trần nhà ít nhất là 8m; tường nhà không được làm bằng vật liệu chói, lóa.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">2. Bộ cột rổ</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">a) Cột và vành rổ được làm bằng kim loại;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">b) Bảng rổ được làm bằng nhựa tổng hợp hoặc bằng gỗ;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">c) Lưới rổ được làm bằng sợi nylon;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">d) Khoảng cách tính từ mép trên của vành rổ xuống mặt sân bóng rổ theo chiều thẳng đứng là 3,050m đối với người tập trên 12 tuổi hoặc ít hơn 3,050m đối với người tập từ 12 tuổi trở xuống.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">3. Quả bóng rổ</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">a) Quả bóng rổ phải được sử dụng phù hợp với từng loại đối tượng theo quy định của Luật thi đấu bóng rổ;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">b) Bảo đảm mỗi người có ít nhất 01 (một) quả bóng rổ.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">4. Độ chiếu sáng bảo đảm trên sân ít nhất là 300 lux.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">5. Có túi sơ cứu theo quy định của Bộ Y tế; có nơi thay đồ, cất giữ đồ và có khu vệ sinh.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">6. Có bảng nội quy quy định những nội dung chủ yếu sau: Giờ tập luyện; đối tượng tham gia tập luyện; trang phục, trang thiết bị khi tham gia tập luyện; các biện pháp bảo đảm trật tự, an toàn trong tập luyện.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(2) Cơ sở vật chất, trang thiết bị thi đấu</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">1. Cơ sở vật chất, trang thiết bị thi đấu môn Bóng rổ được thực hiện theo quy định tại các mục 1, 2, 4, 5 và điểm a mục 3 phần (1) nêu trên.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">2. Mặt sân phải được làm bằng gỗ hoặc chất liệu tổng hợp.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">3. Mỗi sân có dụng cụ lau và làm sạch mặt sân.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(3) Mật độ hướng dẫn tập luyện</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">1. Mỗi người hướng dẫn tập luyện hướng dẫn không quá 30 người trong một buổi tập.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">2. Mật độ tập luyện được bảo đảm ít nhất 10m2/người.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(4) Nhân viên chuyên môn</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">a) Đối với cơ sở kinh doanh hoạt động thể thao thuộc một trong các trường hợp:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Cung cấp dịch vụ hướng dẫn tập luyện thể thao;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Kinh doanh hoạt động thể thao thuộc Danh mục hoạt động thể thao bắt buộc có hướng dẫn tập luyện. Danh mục hoạt động thể thao bắt buộc có hướng dẫn tập luyện do Bộ trưởng Bộ Văn hóa, Thể thao và Du lịch quy định.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Phải có người hướng dẫn tập luyện thể thao đáp ứng một trong các điều kiện sau:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Là huấn luyện viên hoặc vận động viên phù hợp với hoạt động thể thao đăng ký kinh doanh có đẳng cấp từ cấp 2 trở lên hoặc tương đương;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Có bằng cấp về chuyên ngành thể dục, thể thao từ bậc trung cấp trở lên phù hợp với hoạt động thể thao đăng ký kinh doanh;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Được tập huấn chuyên môn thể thao theo quy định của Bộ trưởng Bộ Văn hóa, Thể thao và Du lịch.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">b) Đối với cơ sở kinh doanh hoạt động thể thao mạo hiểm phải có đủ nhân viên chuyên môn sau đây:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Người hướng dẫn tập luyện thể thao đáp ứng một trong các điều kiện sau:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Là huấn luyện viên hoặc vận động viên phù hợp với hoạt động thể thao đăng ký kinh doanh có đẳng cấp từ cấp 2 trở lên hoặc tương đương;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Có bằng cấp về chuyên ngành thể dục, thể thao từ bậc trung cấp trở lên phù hợp với hoạt động thể thao đăng ký kinh doanh;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Được tập huấn chuyên môn thể thao theo quy định của Bộ trưởng Bộ Văn hóa, Thể thao và Du lịch.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Nhân viên cứu hộ.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Nhân viên y tế thường trực hoặc văn bản thỏa thuận với cơ sở y tế gần nhất để sơ cứu, cấp cứu người tham gia hoạt động thể thao mạo hiểm trong trường hợp cần thiết.</span></span></div>
