<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Bước 1: Doanh nghiệp đề nghị cấp giấy chứng nhận đủ điều kiện kinh doanh hoạt động thể thao gửi hồ sơ đến Trung tâm Phục vụ hành chính công tỉnh.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Địa chỉ: 70 Lê Hồng Phong, thành phố Kon Tum.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Thời gian: Vào giờ hành chính từ thứ 2 đến thứ 6 hàng tuần.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Bước 2: Cán bộ tiếp nhận kiểm tra tính hợp lý và đầy đủ của hồ sơ:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Trường hợp hồ sơ đầy đủ thì công chức viết giấy hẹn </span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Trường hợp hồ sơ còn thiếu và không hợp lệ thì công chức hướng dẫn về chỉnh sửa, bổ sung.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Trường hợp hồ sơ cần sửa đổi, bổ sung Sở Văn hóa, Thể thao và Du lịch thông báo trực tiếp hoặc bằng văn bản những nội dung cần sửa đổi, bổ sung đến doanh nghiệp trong thời hạn 03 ngày làm việc, kể từ ngày nhận hồ sơ.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Trong thời hạn bảy ngày làm việc, kể từ ngày nhận đủ hồ sơ hợp lệ, Sở Văn hóa, Thể thao và Du lịch phải kiểm tra các điều kiện kinh doanh hoạt động thể thao của doanh nghiệp theo quy định và cấp giấy chứng nhận đủ điều kiện kinh doanh hoạt động thể thao.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Trường hợp không cấp, Sở Văn hóa, Thể thao và Du lịch trả lời bằng văn bản và nêu rõ lý do.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Bước 3: Trả kết quả tại Trung tâm Phục vụ hành chính công tỉnh hoặc qua đường bưu điện.</span></span></div>

<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">1. Sân tập, sàn tập bằng phẳng, không trơn trượt, có diện tích từ 60m2 trở lên đối với nội dung đối kháng (Sanshou) và 80 m2 trở lên đối với nội dung  quyền (Taolu).</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">2. Độ chiếu sáng sân tập, sàn tập bảo đảm từ 200 lux trở lên.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">3. Có túi sơ cứu theo quy định của Bộ Y tế.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">4. Có khu vực thay đồ, tủ gửi đồ, nhà vệ sinh.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">5. Có sổ theo dõi võ sinh tham gia tập luyện.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">6. Có trang thiết bị tập luyện gồm:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">a) Thảm tập có độ dày ít nhất là 2cm và có tính đàn hồi;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">b) Võ phục, găng, giáp, mũ, kuki trong nội dung đối kháng;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">c) Võ phục, các loại vũ khí thể thao thô sơ: Kiếm, đao, côn, thương đối với các bài quyền như Kiếm thuật, Đao thuật, Côn thuật, Thương thuật, Nam đao, Nam côn, Thái cực kiếm trong nội dung quyền.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Việc đăng ký, sử dụng kiếm, đao, côn, thương thực hiện theo quy định của pháp luật về quản lý, sử dụng vũ khí, vật liệu nổ và công cụ hỗ trợ.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">7. Có bảng nội quy quy định những nội dung chủ yếu: Đối tượng tham gia tập luyện, trang phục khi tập luyện, giờ tập luyện, biện pháp đảm bảo an toàn khi tập luyện.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(2) Cơ sở vật chất, trang thiết bị thi đấu</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">1. Thực hiện quy định tại các mục 2, 3, 4 phần (1) nêu trên.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">2. Sàn thi đấu:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">a) Đối với nội dung Taolu: Thảm đấu có tính đàn hồi, kích thước 18m x 12m x 0,1m;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">b) Đối với nội dung Sanshou: Đài thi đấu có diện tích 8m x 8m, cao 0,8m, có đệm bảo vệ xung quanh.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">3. Trang thiết bị thi đấu:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">a) Thực hiện quy định tại điểm b, c mục 6 phần (1) nêu trên;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">b) Đối với nội dung Taolu: Có bảng công bố điểm;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">c) Đối với nội dung Sanshou: Có cân trọng lượng cơ thể võ sinh.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(3) Mật độ tập luyện</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">1. Mật độ tập luyện trên sàn bảo đảm ít nhất 3m2/01 võ sinh.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">2. Mỗi người hướng dẫn tập luyện hướng dẫn không quá 30 võ sinh/buổi tập.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";"> (4) Nhân viên chuyên môn</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">a) Đối với cơ sở kinh doanh hoạt động thể thao thuộc một trong các trường hợp:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Cung cấp dịch vụ hướng dẫn tập luyện thể thao;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Kinh doanh hoạt động thể thao thuộc Danh mục hoạt động thể thao bắt buộc có hướng dẫn tập luyện. Danh mục hoạt động thể thao bắt buộc có hướng dẫn tập luyện do Bộ trưởng Bộ Văn hóa, Thể thao và Du lịch quy định.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Phải có người hướng dẫn tập luyện thể thao đáp ứng một trong các điều kiện sau:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Là huấn luyện viên hoặc vận động viên phù hợp với hoạt động thể thao đăng ký kinh doanh có đẳng cấp từ cấp 2 trở lên hoặc tương đương;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Có bằng cấp về chuyên ngành thể dục, thể thao từ bậc trung cấp trở lên phù hợp với hoạt động thể thao đăng ký kinh doanh;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Được tập huấn chuyên môn thể thao theo quy định của Bộ trưởng Bộ Văn hóa, Thể thao và Du lịch.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">b) Đối với cơ sở kinh doanh hoạt động thể thao mạo hiểm phải có đủ nhân viên chuyên môn sau đây:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Người hướng dẫn tập luyện thể thao đáp ứng một trong các điều kiện sau:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Là huấn luyện viên hoặc vận động viên phù hợp với hoạt động thể thao đăng ký kinh doanh có đẳng cấp từ cấp 2 trở lên hoặc tương đương;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Có bằng cấp về chuyên ngành thể dục, thể thao từ bậc trung cấp trở lên phù hợp với hoạt động thể thao đăng ký kinh doanh;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Được tập huấn chuyên môn thể thao theo quy định của Bộ trưởng Bộ Văn hóa, Thể thao và Du lịch.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Nhân viên cứu hộ.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Nhân viên y tế thường trực hoặc văn bản thỏa thuận với cơ sở y tế gần nhất để sơ cứu, cấp cứu người tham gia hoạt động thể thao mạo hiểm trong trường hợp cần thiết.</span></span></div>
