<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Bước 1: Tổ chức thuộc cơ quan địa phương đề nghị cấp giấy phép muốn lưu hành hoặc nhập khẩu bản ghi âm, ghi hình có nội dung ca múa nhạc, sân khấu gửi hồ sơ đến Trung tâm Phục vụ hành chính công tỉnh.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Địa chỉ: 70 Lê Hồng Phong, thành phố Kon Tum.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Thời gian: Trong giờ hành chính, từ thứ hai đến thứ sáu hàng tuần.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Bước 2: Cán bộ tiếp nhận kiểm tra tính hợp lý và đầy đủ của hồ sơ:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Trường hợp hồ sơ đầy đủ thì công chức viết giấy hẹn </span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Trường hợp hồ sơ còn thiếu và không hợp lệ thì công chức hướng dẫn về chỉnh sửa, bổ sung.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";"> Bước 3: Sở Văn hóa, Thể thao và Du lịch giúp UBND tỉnh thẩm định hồ sơ và trình UBND tỉnh cấp giấy phép theo thẩm quyền. Trong thời hạn 07 ngày làm việc kể từ ngày nhận đầy đủ hồ sơ hợp lệ, UBND tỉnh cấp giấy phép, trường hợp không cấp giấy phép phải có văn bản trả lời, nêu rõ lý do.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Bước 4: Trả kết quả tại Trung tâm Phục vụ hành chính công tỉnh hoặc qua đường bưu điện.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Thời gian: Trong giờ hành chính, từ thứ hai đến thứ sáu hàng tuần.</span></span></div>

<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Đơn đề nghị cấp giấy phép phê duyệt nội dung (theo mẫu);</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Danh mục tác giả, tác phẩm, người biểu diễn;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Bản nhạc và lời hoặc kịch bản văn học (đối với tác phẩm nước ngoài phải có bản dịch tiếng Việt có chứng nhận của công ty dịch thuật);</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Bản ghi âm, ghi hình có nội dung ca múa nhạc, sân khấu;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Văn bản cam kết thực thi đầy đủ các quy định của pháp luật về quyền tác giả hoặc bản sao hợp đồng hoặc văn bản thoả thuận với tác giả hoặc chủ sở hữu quyền tác giả (theo mẫu);</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Bản sao chứng thực quyết định phổ biến tác phẩm và quyết định cho người Việt Nam định cư ở nước ngoài tham gia biểu diễn nghệ thuật (nếu trong chương trình có sử dụng tác phẩm sáng tác trước năm 1975, tác phẩm do người Việt Nam định cư ở nước ngoài sáng tác và nghệ sĩ là người Việt Nam định cư ở nước ngoài tham gia biểu diễn);</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Mẫu thiết kế vỏ bản ghi âm, ghi hình;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Bản sao quyết định thành lập hoặc giấy chứng nhận đăng ký doanh nghiệp (Đối tượng lần đầu thực hiện thủ tục hành chính và đối tượng trong quá trình hoạt động đã có hành vi vi phạm quy định tại Điều 6 Nghị định số 79/2012/NĐ-CP (có phụ lục đính kèm) nộp bản sao có chứng thực hoặc mang bản gốc để đối chiếu).</span></span></div>

<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Mức thu phí thẩm định nội dung chương trình trên băng, đĩa, phần mềm và trên vật liệu khác như sau:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">a) Chương trình ca múa nhạc, sân khấu ghi trên băng đĩa:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Đối với bản ghi âm: 200.000 đồng/1 block thứ nhất cộng (+) mức phí tăng thêm là 150.000 đồng cho mỗi block tiếp theo (Một block có độ dài thời gian là 15 phút).</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Đối với bản ghi hình: 300.000 đồng/1 block thứ nhất cộng (+) mức phí tăng thêm là 200.000 đồng cho mỗi block tiếp theo (Một block có độ dài thời gian là 15 phút).</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">b) Chương trình ghi trên đĩa nén, ổ cứng, phần mềm và các vật liệu khác:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Đối với bản ghi âm:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Ghi dưới hoặc bằng 50 bài hát, bản nhạc: 2.000.000 đồng/chương trình;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Ghi trên 50 bài hát, bản nhạc: 2.000.000 đồng/chương trình cộng (+) mức phí tăng thêm là 50.000 đồng/bài hát, bản nhạc. Tổng mức phí không quá 7.000.000 đồng/chương trình.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Đối với bản ghi hình:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Ghi dưới hoặc bằng 50 bài hát, bản nhạc: 2.500.000 đồng/chương trình;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Ghi trên 50 bài hát, bản nhạc: 2.500.000 đồng/chương trình cộng (+) mức phí tăng thêm là 75.000 đồng/bài hát, bản nhạc. Tổng mức phí không quá 9.000.000 đồng/chương trình.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Chương trình trên băng, đĩa, phần mềm và trên vật liệu khác sau khi thẩm định không đủ điều kiện cấp giấy phép thì không được hoàn trả số phí thẩm định đã nộp.</span></span></div>
