<div> <p> </p> <span style="font-size: medium;"><span style="font-family: "Times New Roman";"> <p><b>Thủ tục</b><b>: </b><b><i>Cấp giấy phép đưa tác phẩm nhiếp ảnh từ Việt Nam ra nước ngoài triển lãm</i></b></p> </span></span> <table border="1" cellspacing="0" cellpadding="0" width="643"> <tbody> <tr> <td width="161" valign="top"> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";"><b>Trình tự thực hiện:</b></span></span></div> </td> <td width="482"> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";"><b><u>Bước 1:</u></b> Tổ chức, cá nhân đề nghị cấp giấy phép đưa tác phẩm nhiếp ảnh từ Việt Nam ra nước ngoài triển lãm (trong các trường hợp không thuộc thẩm quyền cấp phép của Cục Mỹ thuật, Nhiếp ảnh và Triển lãm) gửi hồ sơ đến Trung tâm Phục vụ hành chính công tỉnh.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Địa chỉ: 70 Lê Hồng Phong, thành phố Kon Tum.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Thời gian: Trong giờ hành chính, từ thứ hai đến thứ sáu hàng tuần.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";"><b><u>Bước 2:</u></b> Cán bộ tiếp nhận kiểm tra tính hợp lý và đầy đủ của hồ sơ:</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Trường hợp hồ sơ đầy đủ thì công chức viết giấy hẹn </span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Trường hợp hồ sơ còn thiếu và không hợp lệ thì công chức hướng dẫn về chỉnh sửa, bổ sung.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";"><b><u>Bước 3:</u></b>Trong thời hạn 07 (bảy) ngày làm việc, kể từ khi nhận đủ hồ sơ hợp lệ, Sở Văn hóa, Thể thao và Du lịch thẩm định hồ sơ và trình Ủy ban nhân dân tỉnh cấp giấy phép, Trường hợp không cấp giấy phép, Sở Văn hóa, Thể thao và Du lịch phải trả lời bằng văn bản, nêu rõ lý do.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Trường hợp cần thành lập Hội đồng thẩm định thì thời hạn cấp giấy phép kéo dài nhưng không quá 10 (mười) ngày làm việc.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Sau khi được cấp phép, nếu có thay đổi một trong các nội dung ghi trong giấy phép, tổ chức, cá nhân tổ chức triển lãm phải làm lại thủ tục cấp phép.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";"><b><u>Bước 4</u></b><b><u>:</u></b> Tổ chức, cá nhân nhận kết quả tại Trung tâm Phục vụ hành chính công tỉnh hoặc qua đường bưu điện.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Thời gian: Trong giờ hành chính, từ thứ hai đến thứ sáu hàng tuần.</span></span></div> </td> </tr> <tr> <td width="161" valign="top"> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";"><b>Cách thức thực hiện:</b></span></span></div> </td> <td width="482"> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Trực tiếp tại trụ sở Trung tâm Phục vụ hành chính công hoặc qua đường bưu điện (hoặc trực tuyến nếu có).</span></span></div> </td> </tr> <tr> <td width="161" valign="top"> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";"><b>Thành phần,</b> <b>số lượng hồ sơ:</b></span></span></div> </td> <td width="482"> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">a) Thành phần hồ sơ; </span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Đơn đề nghị cấp Giấy phép đưa ảnh từ Việt Nam ra nước ngoài triển lãm <i>(theo mẫu)</i>;</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Danh sách tác phẩm nhiếp ảnh bằng tiếng Việt (ghi rõ số thứ tự tác phẩm, tên tác phẩm, tên tác giả, chất liệu, kích thước, số lượng).;</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Ảnh sẽ triển lãm có chú thích;
