<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";"><b><u>Bước 1:</u></b> Tổ chức, cá nhân đề nghị cấp phép sao chép tác phẩm mỹ thuật về danh nhân văn hóa, anh hùng dân tộc, lãnh tụ gửi hồ sơ đến Trung tâm Phục vụ hành chính công tỉnh.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Địa chỉ: 70 Lê Hồng Phong, thành phố Kon Tum.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Thời gian: Trong giờ hành chính, từ thứ hai đến thứ sáu hàng tuần.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";"><b><u>Bước 2:</u></b> Cán bộ tiếp nhận kiểm tra tính hợp lý và đầy đủ của hồ sơ:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Trường hợp hồ sơ đầy đủ thì công chức viết giấy hẹn </span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Trường hợp hồ sơ còn thiếu và không hợp lệ thì công chức hướng dẫn về chỉnh sửa, bổ sung.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";"><b><u>Bước 3:</u></b>. Trong thời hạn 07 ngày làm việc kể từ ngày nhận đầy đủ hồ sơ hợp lệ, Sở Văn hóa, Thể thao và Du lịch có trách nhiệm cấp giấy phép sao chép tác phẩm mỹ thuật về danh nhân văn hóa, anh hùng dân tộc, lãnh tụ cho tổ chức, cá nhân. Trường hợp không cấp giấy phép phải có văn bản trả lời, nêu rõ lý do.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";"><b><u>Bước 4</u></b><b><u>:</u></b> Trả kết quả tại Trung tâm Phục vụ hành chính công tỉnh hoặc qua đường bưu điện</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Thời gian: Trong giờ hành chính, từ thứ hai đến thứ sáu hàng tuần.</span></span></div>

<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Đơn đề nghị cấp giấy phép <i>(</i><i>theo </i><i>mẫu)</i>;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Ảnh màu kích thước 18x24 cm chụp bản mẫu và bản sao;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Bản sao chứng thực hợp đồng sử dụng tác phẩm hoặc văn bản đồng ý của chủ sở hữu tác phẩm mẫu; Nộp bản sao có chứng thực hoặc bản sao có xuất trình bản chính để đối chiếu <i>(trường hợp nộp hồ sơ trực tiếp)</i>, nộp bản sao có chứng thực (<i>trường hợp nộp hồ sơ qua đường bưu điện).</i></span></span></div>
