<p><span style="font-size: medium;"><span style="font-family: "Times New Roman";"><b>Thủ tục: <i>Cấp lại chứng chỉ hành nghề bảo quản, tu bổ, phục hồi di tích</i> </b><b> </b></span></span></p>

<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";"><b>Bước 1:</b> Cá nhân đề nghị cấp lại Chứng chỉ hành nghề bảo quản, tu bổ, phục hồi di tích (sau đây gọi chung là Chứng chỉ hành nghề) gửi hồ sơ đến Trung tâm Phục vụ hành chính công tỉnh.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Địa chỉ: 70 Lê Hồng Phong, thành phố Kon Tum.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Thời gian: Trong giờ hành chính, từ thứ hai đến thứ sáu hàng tuần.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";"> <b>Bước 2:</b> Đối với trường hợp cấp lại Chứng chỉ hành nghề hết hạn sử dụng, bị hỏng, bị lỗi, trong thời hạn 05 (năm) ngày làm việc, kể từ ngày nhận đủ hồ sơ hợp lệ, Giám đốc Sở Văn hóa, Thể thao và Du lịch xem xét, quyết định cấp lại Chứng chỉ hành nghề. Trường hợp không cấp phải nêu rõ lý do bằng văn bản.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";"><b>Bước 3:</b> Tổ chức, cá nhân nhận kết quả tại Trung tâm Phục vụ hành chính công tỉnh hoặc qua đường bưu điện.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Thời gian: Trong giờ hành chính, từ thứ hai đến thứ sáu hàng tuần.</span></span></div>

<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Đơn đề nghị <i>(</i><i>theo </i><i>m</i><i>ẫu</i><i>)</i>;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Bản chính Giấy chứng nhận hành nghề đã được cấp đối với trường hợp bổ sung nội dung hành nghề hoặc Giấy chứng nhận hành nghề hết hạn sử dụng, bị hỏng;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Trường hợp bổ sung nội dung hành nghề đã được ghi nhận trong Giấy chứng nhận hành nghề đã cấp, phải gửi kèm theo hồ sơ các bản sao:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Danh sách người được cấp Chứng chỉ hành nghề kèm theo bản sao Chứng chỉ hành nghề: lập quy hoạch tu bổ di tích; lập dự án tu bổ di tích, báo cáo kinh tế - kỹ thuật tu bổ di tích, thiết kế tu bổ di tích; thi công tu bổ di tích; giám sát thi công tu bổ di tích.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Bản sao Quyết định thành lập hoặc Giấy chứng nhận đăng ký doanh nghiệp hoặc Giấy đăng ký kinh doanh.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">* Đối với các trường hợp cấp lại Chứng chỉ hành nghề bị mất hoặc bổ sung nội dung hành nghề thì thời hạn cấp lại như cấp mới.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">b) Số lượng hồ sơ: 01 bộ.</span></span></div>

<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";"><b>Thời hạn giải quyết:</b></span></span></div>

<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Đối với trường hợp cấp lại hết hạn sử dụng hoặc bị hỏng, trong thời hạn 05 ngày làm việc, kể từ ngày nhận đủ hồ sơ hợp lệ.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Đối với trường hợp cấp lại bị mất hoặc bổ sung nội dung hành nghề, thời hạn cấp được thực hiện như quy định đối với trường hợp cấp mới.</span></span></div>

Trường hợp cấp lại Chứng chỉ hành nghề bị lỗi thì Bộ trưởng Bộ Văn hóa, Thể thao và Du lịch cấp miễn phí theo yêu cầu của cá nhân.</span></span></div>
