10.Cấp lại Giấy chứng nhận đủ điều kiện kinh doanh giám định cổ vật

<p><span style="font-size: medium;"><span style="font-family: "Times New Roman";"><b>Thủ tục: <i>Cấp lại Giấy chứng nhận đủ điều kiện kinh doanh giám định cổ vật</i></b></span></span></p> <table border="1" cellspacing="0" cellpadding="0" width="643"> <tbody> <tr> <td width="170" valign="top"> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";"><b>Trình tự thực hiện:</b></span></span></div> </td> <td width="472" valign="top"> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";"><b>Bước 1:</b> Cơ sở giám định cổ vật đề nghị cấp lại Giấy chứng nhận đủ điều kiện hoạt động giám định cổ vật gửi hồ sơ đến Trung tâm Phục vụ hành chính công tỉnh.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Địa chỉ: 70 Lê Hồng Phong, thành phố Kon Tum.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Thời gian: Trong giờ hành chính, từ thứ hai đến thứ sáu hàng tuần.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";"><b>Bước 2:</b> Cán bộ tiếp nhận kiểm tra tính hợp lý và đầy đủ của hồ sơ:</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Trường hợp hồ sơ đầy đủ thì công chức viết giấy hẹn cho tổ chức</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Trường hợp hồ sơ còn thiếu và không hợp lệ thì công chức hướng dẫn về chỉnh sửa, bổ sung.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Trong thời hạn 05 (năm) ngày làm việc, kể từ ngày nhận đủ hồ sơ hợp lệ, Giám đốc Sở Văn hóa, Thể thao và Du lịch xem xét, quyết định cấp lại Giấy chứng nhận đủ điều kiện hoạt động giám định cổ vật. Trong trường hợp từ chối phải trả lời bằng văn bản và nêu rõ lý do.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";"><b>Bước 3:</b> Trả kết quả tại Trung tâm Phục vụ hành chính công tỉnh hoặc qua đường bưu điện.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Thời gian: Trong giờ hành chính, từ thứ hai đến thứ sáu hàng tuần.</span></span></div> </td> </tr> <tr> <td width="170" valign="top"> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";"><b>Cách "Times New Roman";">a)<b> </b>Thành phần hồ sơ:</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Đơn đề nghị cấp lại Giấy chứng nhận đủ điều kiện hoạt động giám định cổ vật <i>(</i><i>t</i><i>heo mẫu</i>);</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Bản chính Giấy chứng nhận đủ điều kiện hoạt động giám định cổ vật đã được cấp (đối với trường hợp giấy chứng nhận bị hỏng, bị lỗi hoặc có sự thay đổi các thông tin);</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">* Trường hợp thay đổi thông tin đã được ghi nhận trong Giấy chứng nhận đủ điều kiện hoạt động giám định cổ vật đã cấp thì phải gửi kèm theo hồ sơ các giấy tờ có liên quan đến sự thay đổi các thông tin (bản chính hoặc bản sao các giấy tờ có liên quan đến sự thay đổi thông tin. </span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">b) Số lượng hồ sơ: 01 (một) bộ.</span></span></div> </td> </tr> <tr> <td width="170" valign="top"> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";"><b>Thời gian giải quyết:</b></span></span></div> </td> <td width="472" valign="top"> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">05 (năm) ngày làm Roman";"><b>Phí, </b><b>Lệ phí:</b></span></span></div> </td> <td width="472" valign="top"> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Cơ sở giám định cổ vật nộp lệ phí theo quy định của Bộ Tài chính tại thời điểm đề nghị cấp lại Giấy chứng nhận đủ điều kiện hoạt động giám định cổ vật.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Trường hợp cấp lại Giấy chứng nhận đủ điều kiện hoạt động giám định cổ vật bị lỗi do Sở Văn hóa, Thể thao và Du lịch đã cấp gây ra thì Giám đốc Sở Văn hóa, Thể thao và Du lịch có trách nhiệm cấp miễn phí theo yêu cầu của Cơ sở giám định cổ vật. </span></span></div> </td> </tr> <tr> <td width="170" valign="top"> <div><span style="font-size: medium;"><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Giấy chứng nhận đủ điều kiện hoạt động giám định cổ vật được cấp lại trong các trường hợp sau:</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Giấy chứng<b> </b>nhận đủ điều kiện hoạt động giám định cổ vật bị mất hoặc bị hỏng (rách, bẩn, phai mờ) không sử dụng được;</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Giấy chứng nhận đủ điều kiện hoạt động giám định cổ vật bị lỗi do Sở Văn hóa, Thể thao và Du lịch đã cấp gây ra;</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Có sự thay đổi các thông tin đã được ghi nhận trong Giấy chứng nhận
