<p><span style="font-size: medium;"><span style="font-family: "Times New Roman";"><strong>Thủ tục: Cấp giấy chứng nhận đủ điều kiện kinh doanh của doanh nghiệp kinh doanh hoạt động thể thao tổ chức hoạt động Lặn biển thể thao giải trí.</strong> </span></span></p>

<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Bước 1: Doanh nghiệp đề nghị cấp giấy chứng nhận đủ điều kiện kinh doanh hoạt động thể thao gửi hồ sơ đến Trung tâm Phục vụ hành chính công tỉnh.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Địa chỉ: 70 Lê Hồng Phong, thành phố Kon Tum.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Thời gian: Vào giờ hành chính từ thứ 2 đến thứ 6 hàng tuần.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Bước 2: Cán bộ tiếp nhận kiểm tra tính hợp lý và đầy đủ của hồ sơ:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Trường hợp hồ sơ đầy đủ thì công chức viết giấy hẹn </span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Trường hợp hồ sơ còn thiếu và không hợp lệ thì công chức hướng dẫn về chỉnh sửa, bổ sung.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Trường hợp hồ sơ cần sửa đổi, bổ sung Sở Văn hóa, Thể thao và Du lịch thông báo trực tiếp hoặc bằng văn bản những nội dung cần sửa đổi, bổ sung đến doanh nghiệp trong thời hạn 03 ngày làm việc, kể từ ngày nhận hồ sơ.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Trong thời hạn bảy ngày làm việc, kể từ ngày nhận đủ hồ sơ hợp lệ, Sở Văn hóa, Thể thao và Du lịch phải kiểm tra các điều kiện kinh doanh hoạt động thể thao của doanh nghiệp theo quy định và cấp giấy chứng nhận đủ điều kiện kinh doanh hoạt động thể thao.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Trường hợp không cấp, Sở Văn hóa, Thể thao và Du lịch trả lời bằng văn bản và nêu rõ lý do.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Bước 3: Trả kết quả tại Trung tâm Phục vụ hành chính công tỉnh hoặc qua đường bưu điện.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Thời gian: Vào giờ hành chính từ thứ 2 đến thứ 6 hàng tuần.</span></span></div>

<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Bản sao Giấy chứng nhận đăng ký doanh nghiệp; bản sao văn bằng, chứng chỉ, giấy chứng nhận của nhân viên chuyên môn đối với cơ sở kinh doanh hoạt động thể thao thuộc Danh mục hoạt động thể thao bắt buộc có hướng dẫn tập luyện hoặc cơ sở kinh doanh hoạt động thể thao mạo hiểm.</span></span></div>

<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Yêu cầu, điều kiện:</span></span></div>

<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(1) Có nguồn tài chính bảo đảm hoạt động kinh doanh hoạt động thể thao. Nguồn tài chính do cơ sở kinh doanh hoạt động thể thao tự chịu trách nhiệm.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(2) Cơ sở vật chất</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">a) Khu vực lặn biển phải có đầy đủ hệ thống phao tiêu, biển báo được định vị phù hợp với tọa đồ trên hải đồ. Phao tiêu, biển báo phải có màu sắc tương phản với màu nước và cảnh quan môi trường để dễ quan sát.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">b) Có phương tiện thủy nội địa vận chuyển người lặn biển, người hướng dẫn tập luyện, nhân viên cứu hộ và các trang thiết bị, dụng cụ môn Lặn biển thể thao giải trí. </span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">c) Có khu vực tập kết phương tiện thủy nội địa và neo đậu phương tiện thủy nội địa.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">d) Có phòng thay đồ, gửi đồ, nhà tắm, khu vực vệ sinh cho người lặn biển; có túi sơ cứu theo quy định của Bộ Y tế.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">đ) Có sổ theo dõi người lặn biển bao gồm những nội dung chủ yếu: Họ và tên, số chứng minh nhân dân hoặc thẻ căn cước công dân, tình trạng sức khỏe của người lặn biển, địa chỉ và số điện thoại liên hệ khi cần thiết.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">e) Có bảng nội quy lặn biển quy định nội dung chủ yếu: Giờ tập luyện, các đối tượng không được tham gia, trang phục khi tham gia, biện pháp đảm bảo an toàn.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(3) Trang thiết bị </span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">a) Trang bị bảo hộ cá nhân dành cho người lặn biển và người hướng dẫn tập luyện: Máy nén khí, bình khí nén, bình ô xy, đồng hồ định vị, đồng hồ đo áp lực, đồng hồ đo độ sâu, đồng hồ đo nhiệt độ, đồng hồ đo thời gian lặn, quần áo lặn, thắt lưng chì, chân vịt, kính lặn, ống thở, hệ thống van, đường ống dẫn khí đến miệng thở, phao cứu sinh.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">b) Hệ thống thông tin liên lạc đảm bảo kết nối liên tục giữa trung tâm tìm kiếm, cứu nạn của địa phương hoặc của quốc gia với các khu vực hoạt động trong phạm vi quản lý của cơ sở.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(4) Bảo đảm an toàn </span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">a) Khi có người lặn dưới biển, phương tiện thủy nội địa tổ chức hoạt động lặn biển phải treo cờ hiệu để báo hiệu đảm bảo an toàn cho người lặn biển. </span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">b) Người hướng dẫn tập luyện phải hướng dẫn người lặn biển biết cách thức sử dụng bình khí, các thiết bị lặn, sử dụng phao cứu sinh và cách thức lặn.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">c) Người lặn biển có trách nhiệm khai báo tình trạng sức khỏe theo quy định của cơ sở thể thao và chịu trách nhiệm về khai báo của mình.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(5) Mật độ hướng dẫn tập luyện</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Mỗi người hướng dẫn tập luyện hướng dẫn không quá 01 người lặn biển trong một lần lặn.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(6) Nhân viên chuyên môn</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">a) Đối với cơ sở kinh doanh hoạt động thể thao thuộc một trong các trường hợp: </span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Cung cấp dịch vụ hướng dẫn tập luyện thể thao;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Kinh doanh hoạt động thể thao thuộc Danh mục hoạt động thể thao bắt buộc có hướng dẫn tập luyện. Danh mục hoạt động thể thao bắt buộc có hướng dẫn tập luyện do Bộ trưởng Bộ Văn hóa, Thể thao và Du lịch quy định.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Phải có người hướng dẫn tập luyện thể thao đáp ứng một trong các điều kiện sau:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Là huấn luyện viên hoặc vận động viên phù hợp với hoạt động thể thao đăng ký kinh doanh có đẳng cấp từ cấp 2 trở lên hoặc tương đương;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Có bằng cấp về chuyên ngành thể dục, thể thao từ bậc trung cấp trở lên phù hợp với hoạt động thể thao đăng ký kinh doanh;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Được tập huấn chuyên môn thể thao theo quy định của Bộ trưởng Bộ Văn hóa, Thể thao và Du lịch.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">b) Đối với cơ sở kinh doanh hoạt động thể thao mạo hiểm phải có đủ nhân viên chuyên môn sau đây:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Người hướng dẫn tập luyện thể thao đáp ứng một trong các điều kiện sau:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Là huấn luyện viên hoặc vận động viên phù hợp với hoạt động thể thao đăng ký kinh doanh có đẳng cấp từ cấp 2 trở lên hoặc tương đương;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Có bằng cấp về chuyên ngành thể dục, thể thao từ bậc trung cấp trở lên phù hợp với hoạt động thể thao đăng ký kinh doanh;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Được tập huấn chuyên môn thể thao theo quy định của Bộ trưởng Bộ Văn hóa, Thể thao và Du lịch.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Nhân viên cứu hộ.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Nhân viên y tế thường trực hoặc văn bản thỏa thuận với cơ sở y tế gần nhất về nhân viên y tế để sơ cứu, cấp cứu người tham gia hoạt động thể thao mạo hiểm trong trường hợp cần thiết.</span></span></div>
