<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Bước 1: Doanh nghiệp đề nghị cấp giấy chứng nhận đủ điều kiện kinh doanh hoạt động thể thao gửi hồ sơ đến Trung tâm Phục vụ hành chính công tỉnh.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Địa chỉ: 70 Lê Hồng Phong, thành phố Kon Tum.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Thời gian: Vào giờ hành chính từ thứ 2 đến thứ 6 hàng tuần.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Bước 2: Cán bộ tiếp nhận kiểm tra tính hợp lý và đầy đủ của hồ sơ:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Trường hợp hồ sơ đầy đủ thì công chức viết giấy hẹn </span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Trường hợp hồ sơ còn thiếu và không hợp lệ thì công chức hướng dẫn về chỉnh sửa, bổ sung.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Trường hợp hồ sơ cần sửa đổi, bổ sung Sở Văn hóa, Thể thao và Du lịch thông báo trực tiếp hoặc bằng văn bản những nội dung cần sửa đổi, bổ sung đến doanh nghiệp trong thời hạn 03 ngày làm việc, kể từ ngày nhận hồ sơ.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Trong thời hạn bảy ngày làm việc, kể từ ngày nhận đủ hồ sơ hợp lệ, Sở Văn hóa, Thể thao và Du lịch phải kiểm tra các điều kiện kinh doanh hoạt động thể thao của doanh nghiệp theo quy định và cấp giấy chứng nhận đủ điều kiện kinh doanh hoạt động thể thao.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Trường hợp không cấp, Sở Văn hóa, Thể thao và Du lịch trả lời bằng văn bản và nêu rõ lý do.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Bước 3: Trả kết quả tại Trung tâm Phục vụ hành chính công tỉnh hoặc qua đường bưu điện.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Thời gian: Vào giờ hành chính từ thứ 2 đến thứ 6 hàng tuần.</span></span></div>

<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Bản sao Giấy chứng nhận đăng ký doanh nghiệp; bản sao văn bằng, chứng chỉ, giấy chứng nhận của nhân viên chuyên môn đối với cơ sở kinh doanh hoạt động thể thao thuộc Danh mục hoạt động thể thao bắt buộc có hướng dẫn tập luyện hoặc cơ sở kinh doanh hoạt động thể thao mạo hiểm.</span></span></div>

<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Yêu cầu, điều kiện:</span></span></div>

<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(1) Có nguồn tài chính bảo đảm hoạt động kinh doanh hoạt động thể thao. Nguồn tài chính do cơ sở kinh doanh hoạt động thể thao tự chịu trách nhiệm.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(2) Cơ sở vật chất, trang thiết bị tập luyện</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">a) Mặt sân phẳng, không trơn trượt, có độ dốc thoát nước và được phủ bằng sơn, cỏ, đất nện hoặc chất liệu tổng hợp.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">b) Sân đơn có chiều dài là 23,77m; chiều rộng là 8,23m. Sân đôi có chiều dài là 23,77m; chiều rộng là 10,97m.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">c) Lưới chắn bóng bao quanh sân, bảo đảm khoảng cách từ mép biên ngang đến lưới chắn ít nhất là 4m và khoảng cách từ mép biên dọc đến lưới chắn ít nhất là 3m.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">d) Độ chiếu sáng trên sân bảo đảm từ 150Lux trở lên.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">đ) Có túi sơ cứu theo quy định của Bộ Y tế; có khu vực vệ sinh, thay đồ và nơi cất giữ đồ dùng cá nhân.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">e) Có bảng nội quy quy định những nội dung chủ yếu sau: Giờ tập luyện; đối tượng không được tham gia tập luyện; biện pháp bảo đảm an toàn khi tập luyện. </span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(3) Cơ sở vật chất, trang thiết bị thi đấu</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">a) Thực thiện theo quy định tại các điểm a, b, c, d và đ mục (2) nêu trên.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">b) Có ghế trọng tài, bảng điểm và dụng cụ đẩy nước. </span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">c) Trường hợp sân thi đấu có mái che, chiều cao từ mặt sân đến điểm thấp nhất của mái che ít nhất là 9m.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(4) Mật độ hướng dẫn tập luyện</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Mỗi người hướng dẫn tập luyện hướng dẫn không quá 12 người trong 01 buổi tập.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(5) Nhân viên chuyên môn</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">a) Đối với cơ sở kinh doanh hoạt động thể thao thuộc một trong các trường hợp: </span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Cung cấp dịch vụ hướng dẫn tập luyện thể thao;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Kinh doanh hoạt động thể thao thuộc Danh mục hoạt động thể thao bắt buộc có hướng dẫn tập luyện. Danh mục hoạt động thể thao bắt buộc có hướng dẫn tập luyện do Bộ trưởng Bộ Văn hóa, Thể thao và Du lịch quy định.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Phải có người hướng dẫn tập luyện thể thao đáp ứng một trong các điều kiện sau:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Là huấn luyện viên hoặc vận động viên phù hợp với hoạt động thể thao đăng ký kinh doanh có đẳng cấp từ cấp 2 trở lên hoặc tương đương;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Có bằng cấp về chuyên ngành thể dục, thể thao từ bậc trung cấp trở lên phù hợp với hoạt động thể thao đăng ký kinh doanh;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Được tập huấn chuyên môn thể thao theo quy định của Bộ trưởng Bộ Văn hóa, Thể thao và Du lịch.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">b) Đối với cơ sở kinh doanh hoạt động thể thao mạo hiểm phải có đủ nhân viên chuyên môn sau đây:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Người hướng dẫn tập luyện thể thao đáp ứng một trong các điều kiện sau:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Là huấn luyện viên hoặc vận động viên phù hợp với hoạt động thể thao đăng ký kinh doanh có đẳng cấp từ cấp 2 trở lên hoặc tương đương;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Có bằng cấp về chuyên ngành thể dục, thể thao từ bậc trung cấp trở lên phù hợp với hoạt động thể thao đăng ký kinh doanh;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Được tập huấn chuyên môn thể thao theo quy định của Bộ trưởng Bộ Văn hóa, Thể thao và Du lịch.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Nhân viên y tế thường trực hoặc văn bản thỏa thuận với cơ sở y tế gần nhất về nhân viên y tế để sơ cứu, cấp cứu người tham gia hoạt động thể thao mạo hiểm trong trường hợp cần thiết.</span></span></div>
