<p><span style="font-size: medium;"><span style="font-family: "Times New Roman";"><strong>Thủ tục: Cấp giấy chứng nhận đủ điều kiện kinh doanh của doanh nghiệp kinh doanh hoạt động thể thao tổ chức hoạt động Quyền anh.</strong> </span></span></p>

<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Bước 1: Doanh nghiệp đề nghị cấp giấy chứng nhận đủ điều kiện kinh doanh hoạt động thể thao gửi hồ sơ đến Trung tâm Phục vụ hành chính công tỉnh.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Địa chỉ: 70 Lê Hồng Phong, thành phố Kon Tum.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Thời gian: Vào giờ hành chính từ thứ 2 đến thứ 6 hàng tuần.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Bước 2: Cán bộ tiếp nhận kiểm tra tính hợp lý và đầy đủ của hồ sơ:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Trường hợp hồ sơ đầy đủ thì công chức viết giấy hẹn </span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Trường hợp hồ sơ còn thiếu và không hợp lệ thì công chức hướng dẫn về chỉnh sửa, bổ sung.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Trường hợp hồ sơ cần sửa đổi, bổ sung Sở Văn hóa, Thể thao và Du lịch thông báo trực tiếp hoặc bằng văn bản những nội dung cần sửa đổi, bổ sung đến doanh nghiệp trong thời hạn 03 ngày làm việc, kể từ ngày nhận hồ sơ.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Trong thời hạn bảy ngày làm việc, kể từ ngày nhận đủ hồ sơ hợp lệ, Sở Văn hóa, Thể thao và Du lịch phải kiểm tra các điều kiện kinh doanh hoạt động thể thao của doanh nghiệp theo quy định và cấp giấy chứng nhận đủ điều kiện kinh doanh hoạt động thể thao.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Trường hợp không cấp, Sở Văn hóa, Thể thao và Du lịch trả lời bằng văn bản và nêu rõ lý do.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Bước 3: Trả kết quả tại Trung tâm Phục vụ hành chính công tỉnh hoặc qua đường bưu điện.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Thời gian: Vào giờ hành chính từ thứ 2 đến thứ 6 hàng tuần.</span></span></div>

<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Bản sao Giấy chứng nhận đăng ký doanh nghiệp; bản sao văn bằng, chứng chỉ, giấy chứng nhận của nhân viên chuyên môn đối với cơ sở kinh doanh hoạt động thể thao thuộc Danh mục hoạt động thể thao bắt buộc có hướng dẫn tập luyện hoặc cơ sở kinh doanh hoạt động thể thao mạo hiểm.</span></span></div>

<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">a) Cơ sở vật chất</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Sàn tập có diện tích ít nhất từ 60m2 trở lên, mặt sàn bằng phẳng, không trơn trượt;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Ánh sáng bảo đảm từ 200 lux trở lên;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Có túi sơ cứu theo quy định của Bộ Y tế;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Có khu vực thay đồ, gửi đồ và nhà vệ sinh;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Có sổ theo dõi võ sinh tham gia tập luyện;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Có bảng nội quy bao gồm những nội dung chủ yếu sau: Đối tượng tham gia tập luyện, trang phục tập luyện, giờ tập luyện, biện pháp bảo đảm an toàn khi tập luyện.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">b) Trang thiết bị</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Găng tập luyện: 01 đôi/01 người;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Mũ bảo vệ: 01 chiếc/01 người;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Bao đấm (bao cát), gối đấm;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Dụng cụ đỡ đòn (lăm pơ);</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Bịt răng, bảo vệ bộ hạ (kuki), băng đa, dây nhảy: 01chiếc/ 01 người.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(3) Cơ sở vật chất, trang thiết bị thi đấu</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">a) Sàn tập có diện tích ít nhất từ 60m2 trở lên, mặt sàn bằng phẳng, không trơn trượt;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">b) Ánh sáng bảo đảm từ 200 lux trở lên;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">c) Có túi sơ cứu theo quy định của Bộ Y tế;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">d) Có khu vực thay đồ, gửi đồ và nhà vệ sinh;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">đ) Găng tập luyện: 01 đôi/01 người;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">e) Mũ bảo vệ: 01 chiếc/01 người;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">g) Bịt răng, bảo vệ bộ hạ (kuki), băng đa, dây nhảy: 01chiếc/ 01 người.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">h) Khu vực thi đấu: Kích thước ít nhất là 06m x 06m, xung quanh có dây bảo vệ, khoảng cách từ dây bảo vệ đến khu vực khán giả ít nhất là 02m.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">i) Các khu vực khác: Cân võ sinh, khởi động và bàn y tế.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">k) Đồng hồ bấm giờ, còi báo hiệu, loa thông báo, bảng điểm, cân trọng lượng cơ thể.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(4) Mật độ tập luyện.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">a) Mật độ tập luyện bảo đảm ít nhất 02m2/01người.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">b) Mỗi người hướng dẫn tập luyện hướng dẫn không quá 30 võ sinh trong một buổi tập.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(5) Nhân viên chuyên môn</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">a) Đối với cơ sở kinh doanh hoạt động thể thao thuộc một trong các trường hợp: </span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Cung cấp dịch vụ hướng dẫn tập luyện thể thao;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Kinh doanh hoạt động thể thao thuộc Danh mục hoạt động thể thao bắt buộc có hướng dẫn tập luyện. Danh mục hoạt động thể thao bắt buộc có hướng dẫn tập luyện do Bộ trưởng Bộ Văn hóa, Thể thao và Du lịch quy định.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Phải có người hướng dẫn tập luyện thể thao đáp ứng một trong các điều kiện sau:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Là huấn luyện viên hoặc vận động viên phù hợp với hoạt động thể thao đăng ký kinh doanh có đẳng cấp từ cấp 2 trở lên hoặc tương đương;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Có bằng cấp về chuyên ngành thể dục, thể thao từ bậc trung cấp trở lên phù hợp với hoạt động thể thao đăng ký kinh doanh;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Được tập huấn chuyên môn thể thao theo quy định của Bộ trưởng Bộ Văn hóa, Thể thao và Du lịch.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">b) Đối với cơ sở kinh doanh hoạt động thể thao mạo hiểm phải có đủ nhân viên chuyên môn sau đây:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Người hướng dẫn tập luyện thể thao đáp ứng một trong các điều kiện sau:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Là huấn luyện viên hoặc vận động viên phù hợp với hoạt động thể thao đăng ký kinh doanh có đẳng cấp từ cấp 2 trở lên hoặc tương đương;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Có bằng cấp về chuyên ngành thể dục, thể thao từ bậc trung cấp trở lên phù hợp với hoạt động thể thao đăng ký kinh doanh;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Được tập huấn chuyên môn thể thao theo quy định của Bộ trưởng Bộ Văn hóa, Thể thao và Du lịch.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Nhân viên y tế thường trực hoặc văn bản thỏa thuận với cơ sở y tế gần nhất về nhân viên y tế để sơ cứu, cấp cứu người tham gia hoạt động thể thao mạo hiểm trong trường hợp cần thiết.</span></span></div>
