<p><span style="font-size: medium;"><span style="font-family: "Times New Roman";"><strong>Thủ tục: Cấp giấy chứng nhận đủ điều kiện kinh doanh của doanh nghiệp kinh doanh hoạt động thể thao tổ chức hoạt động Thể dục thể hình và Fitness.</strong> </span></span></p>

<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Bước 1: Doanh nghiệp đề nghị cấp giấy chứng nhận đủ điều kiện kinh doanh hoạt động thể thao gửi hồ sơ đến Trung tâm Phục vụ hành chính công tỉnh.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Địa chỉ: 70 Lê Hồng Phong, thành phố Kon Tum.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Thời gian: Vào giờ hành chính từ thứ 2 đến thứ 6 hàng tuần.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Bước 2: Cán bộ tiếp nhận kiểm tra tính hợp lý và đầy đủ của hồ sơ:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Trường hợp hồ sơ đầy đủ thì công chức viết giấy hẹn </span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Trường hợp hồ sơ còn thiếu và không hợp lệ thì công chức hướng dẫn về chỉnh sửa, bổ sung.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Trường hợp hồ sơ cần sửa đổi, bổ sung Sở Văn hóa, Thể thao và Du lịch thông báo trực tiếp hoặc bằng văn bản những nội dung cần sửa đổi, bổ sung đến doanh nghiệp trong thời hạn 03 ngày làm việc, kể từ ngày nhận hồ sơ.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Trong thời hạn bảy ngày làm việc, kể từ ngày nhận đủ hồ sơ hợp lệ, Sở Văn hóa, Thể thao và Du lịch phải kiểm tra các điều kiện kinh doanh hoạt động thể thao của doanh nghiệp theo quy định và cấp giấy chứng nhận đủ điều kiện kinh doanh hoạt động thể thao.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Trường hợp không cấp, Sở Văn hóa, Thể thao và Du lịch trả lời bằng văn bản và nêu rõ lý do.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Bước 3: Trả kết quả tại Trung tâm Phục vụ hành chính công tỉnh hoặc qua đường bưu điện.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Thời gian: Vào giờ hành chính từ thứ 2 đến thứ 6 hàng tuần.</span></span></div>

<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Bản sao Giấy chứng nhận đăng ký doanh nghiệp; bản sao văn bằng, chứng chỉ, giấy chứng nhận của nhân viên chuyên môn đối với cơ sở kinh doanh hoạt động thể thao thuộc Danh mục hoạt động thể thao bắt buộc có hướng dẫn tập luyện hoặc cơ sở kinh doanh hoạt động thể thao mạo hiểm.</span></span></div>

<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(1) Có nguồn tài chính bảo đảm hoạt động kinh doanh hoạt động thể thao. Nguồn tài chính do cơ sở kinh doanh hoạt động thể thao tự chịu trách nhiệm</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(2) Cơ sở vật chất, trang thiết bị tập luyện</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">a) Địa điểm tập luyện:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Phòng tập luyện diện tích ít nhất 60 m2, khoảng cách từ sàn đến trần ít nhất 2,8m, không gian tập luyện phải bảo đảm thông thoáng. Khoảng cách giữa các trang thiết bị tập luyện đảm bảo từ 10cm đến 30cm.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Ánh sáng từ 150 lux trở lên;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Hệ thống âm thanh trong tình trạng hoạt động tốt;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Có khu vực vệ sinh, thay đồ, nơi để đồ dùng cá nhân cho người tập; có túi sơ cứu theo quy định của Bộ Y tế.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Nội quy bao gồm những nội dung chủ yếu sau: Giờ tập luyện, đối tượng tham gia tập luyện, các đối tượng không được tham gia tập luyện, biện pháp đảm bảo an toàn khi tập luyện.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">b) Trang thiết bị tập luyện phải đảm bảo theo quy định tại Phụ lục 1 ban hành kèm theo Thông tư số 10/2018/TT-BVHTTDL ngày 31/01/2018 của Bộ trưởng Bộ Văn hóa, Thể thao và Du lịch (đính kèm sau TTHC này).</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(3) Cơ sở vật chất, trang thiết bị thi đấu</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">a) Địa điểm thi đấu môn Thể dục thể hình và Fitness diễn ra trong nhà hoặc ngoài trời.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">b) Sân khấu: Kích thước sân khấu 12m x 12m và có chiều cao 0,8m. Mặt sân khấu phải được trải thảm mềm. Thảm mầu xanh nước biển hoặc mầu xanh ngọc.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">c) Phông: Phía sau sân khấu thi đấu phải căng một tấm phông với một trong những mầu sắc sau đây: Xanh đen, nâu sẫm, xanh lục sẫm, tím sẫm.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">d) Bục thi đấu môn Thể dục thể hình đặt ở vị trí trung tâm sân khấu. Kích thước bục dài 06m, rộng 02m và cao 0,3m, có thảm bao xung quanh mầu lục nhạt hoặc mầu lam nhạt.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">đ) Ánh sáng từ 1500lux trở lên. Đối với môn Fitness phải sáng đều khắp sân khấu; đối với môn Thể dục thể hình ánh sáng tập trung chiếu rọi khu vực bục thi đấu.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">e) Hệ thống âm thanh trong tình trạng hoạt động tốt.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">g) Khu vực khởi động phải gần địa điểm thi đấu. Trang thiết bị khởi động cần có gồm: Các cần tạ, bánh tạ, tạ tay, dây chun, dây lò so, các ghế tập.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(4) Mật độ hướng dẫn tập luyện: Mỗi người hướng dẫn tập luyện chỉ được hướng dẫn không quá 70 người trong một buổi tập. </span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(5) Nhân viên chuyên môn</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">a) Đối với cơ sở kinh doanh hoạt động thể thao thuộc một trong các trường hợp: </span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Cung cấp dịch vụ hướng dẫn tập luyện thể thao;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Kinh doanh hoạt động thể thao thuộc Danh mục hoạt động thể thao bắt buộc có hướng dẫn tập luyện. Danh mục hoạt động thể thao bắt buộc có hướng dẫn tập luyện do Bộ trưởng Bộ Văn hóa, Thể thao và Du lịch quy định.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Phải có người hướng dẫn tập luyện thể thao đáp ứng một trong các điều kiện sau:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Là huấn luyện viên hoặc vận động viên phù hợp với hoạt động thể thao đăng ký kinh doanh có đẳng cấp từ cấp 2 trở lên hoặc tương đương;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Có bằng cấp về chuyên ngành thể dục, thể thao từ bậc trung cấp trở lên phù hợp với hoạt động thể thao đăng ký kinh doanh;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Được tập huấn chuyên môn thể thao theo quy định của Bộ trưởng Bộ Văn hóa, Thể thao và Du lịch.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">b) Đối với cơ sở kinh doanh hoạt động thể thao mạo hiểm phải có đủ nhân viên chuyên môn sau đây:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Người hướng dẫn tập luyện thể thao đáp ứng một trong các điều kiện sau:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Là huấn luyện viên hoặc vận động viên phù hợp với hoạt động thể thao đăng ký kinh doanh có đẳng cấp từ cấp 2 trở lên hoặc tương đương;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Có bằng cấp về chuyên ngành thể dục, thể thao từ bậc trung cấp trở lên phù hợp với hoạt động thể thao đăng ký kinh doanh;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Được tập huấn chuyên môn thể thao theo quy định của Bộ trưởng Bộ Văn hóa, Thể thao và Du lịch.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Nhân viên y tế thường trực hoặc văn bản thỏa thuận với cơ sở y tế gần nhất về nhân viên y tế để sơ cứu, cấp cứu người tham gia hoạt động thể thao mạo hiểm trong trường hợp cần thiết.</span></span></div>
