<p><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Thủ tục: Cấp giấy chứng nhận đủ điều kiện kinh doanh của doanh nghiệp kinh doanh hoạt động thể thao tổ chức hoạt động Bóng bàn. </span></span></p>

<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Bước 1: Doanh nghiệp đề nghị cấp giấy chứng nhận đủ điều kiện kinh doanh hoạt động thể thao gửi hồ sơ đến Trung tâm Phục vụ hành chính công tỉnh.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Địa chỉ: 70 Lê Hồng Phong, thành phố Kon Tum.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Thời gian: Vào giờ hành chính từ thứ 2 đến thứ 6 hàng tuần.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Bước 2: Cán bộ tiếp nhận kiểm tra tính hợp lý và đầy đủ của hồ sơ:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Trường hợp hồ sơ đầy đủ thì công chức viết giấy hẹn </span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Trường hợp hồ sơ còn thiếu và không hợp lệ thì công chức hướng dẫn về chỉnh sửa, bổ sung.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Trường hợp hồ sơ cần sửa đổi, bổ sung Sở Văn hóa, Thể thao và Du lịch thông báo trực tiếp hoặc bằng văn bản những nội dung cần sửa đổi, bổ sung đến doanh nghiệp trong thời hạn 03 ngày làm việc, kể từ ngày nhận hồ sơ.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Trong thời hạn bảy ngày làm việc, kể từ ngày nhận đủ hồ sơ hợp lệ, Sở Văn hóa, Thể thao và Du lịch phải kiểm tra các điều kiện kinh doanh hoạt động thể thao của doanh nghiệp theo quy định và cấp giấy chứng nhận đủ điều kiện kinh doanh hoạt động thể thao.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Trường hợp không cấp, Sở Văn hóa, Thể thao và Du lịch trả lời bằng văn bản và nêu rõ lý do.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Bước 3: Trả kết quả tại Trung tâm Phục vụ hành chính công tỉnh hoặc qua đường bưu điện.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Thời gian: Vào giờ hành chính từ thứ 2 đến thứ 6 hàng tuần.</span></span></div>

<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Bản sao Giấy chứng nhận đăng ký doanh nghiệp; bản sao văn bằng, chứng chỉ, giấy chứng nhận của nhân viên chuyên môn đối với cơ sở kinh doanh hoạt động thể thao thuộc Danh mục hoạt động thể thao bắt buộc có hướng dẫn tập luyện hoặc cơ sở kinh doanh hoạt động thể thao mạo hiểm.</span></span></div>

<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(1) Có nguồn tài chính bảo đảm hoạt động kinh doanh hoạt động thể thao. Nguồn tài chính do cơ sở kinh doanh hoạt động thể thao tự chịu trách nhiệm</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(2) Cơ sở vật chất, trang thiết bị tập luyện</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">a) Khu vực đặt bàn phải có mái che, kín gió, không bị chói mắt. Sàn bằng phẳng, không trơn trượt.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">b) Bàn bóng được đặt trong khuôn viên có kích thước chiều rộng ít nhất 5m, chiều dài ít nhất 8m.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">c) Mặt bàn có độ nẩy đồng đều khoảng 23cm khi để quả bóng tiêu chuẩn rơi từ độ cao 30cm xuống mặt bàn.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">d) Quả bóng hình cầu có đường kính 40mm, nặng 2,7g làm bằng chất liệu xen-lu-lô-ít hoặc chất liệu nhựa dẻo tương tự, có màu trắng hoặc màu da cam.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">đ) Lưới và cọc lưới có chiều cao 15.25cm. Khoảng cách giới hạn ngoài đường biên dọc với cọc lưới là 15.25cm, mép trên của lưới phải cao đều 15.25cm, mép dưới của lưới phải sát với mặt bàn, cạnh bên của lưới phải sát với cọc lưới.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">e) Bảo đảm ánh sáng đồng đều tới các điểm trên mặt bàn và khu vực bàn bóng ít nhất 300 Lux, đèn được thiết kế cho mỗi bàn có chiều cao ít nhất tính từ mặt bàn là 4m.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">g) Tấm chắn bóng quanh khuôn viên đặt bàn cao 75cm, sẫm màu, tránh phản quang và lẫn với màu của quả bóng.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">h) Có túi sơ cứu theo quy định của Bộ Y tế, nơi thay đồ, cất giữ đồ và khu vực vệ sinh.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">i) Có bảng nội quy quy định những nội dung chủ yếu sau: Giờ tập luyện, đối tượng tham gia tập luyện, các đối tượng không được tham gia tập luyện, biện pháp bảo đảm an toàn khi tập luyện.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(3) Cơ sở vật chất, trang thiết bị thi đấu</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">a) Thực hiện theo quy định tại các điểm a, c, d, đ, e, g và h mục (2) nêu trên.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">b) Bàn bóng được đặt trong khuôn viên có kích thước chiều rộng ít nhất 7m, chiều dài ít nhất 14m.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">c) Có bàn để bảng lật số, ghế trọng tài.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(4) Mật độ tập luyện: Mỗi người hướng dẫn tập luyện hướng dẫn không quá 30 người trong một buổi tập.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(5) Nhân viên chuyên môn</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">a) Đối với cơ sở kinh doanh hoạt động thể thao thuộc một trong các trường hợp: </span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Cung cấp dịch vụ hướng dẫn tập luyện thể thao;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Kinh doanh hoạt động thể thao thuộc Danh mục hoạt động thể thao bắt buộc có hướng dẫn tập luyện. Danh mục hoạt động thể thao bắt buộc có hướng dẫn tập luyện do Bộ trưởng Bộ Văn hóa, Thể thao và Du lịch quy định.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Phải có người hướng dẫn tập luyện thể thao đáp ứng một trong các điều kiện sau:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Là huấn luyện viên hoặc vận động viên phù hợp với hoạt động thể thao đăng ký kinh doanh có đẳng cấp từ cấp 2 trở lên hoặc tương đương;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Có bằng cấp về chuyên ngành thể dục, thể thao từ bậc trung cấp trở lên phù hợp với hoạt động thể thao đăng ký kinh doanh;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Được tập huấn chuyên môn thể thao theo quy định của Bộ trưởng Bộ Văn hóa, Thể thao và Du lịch.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">b) Đối với cơ sở kinh doanh hoạt động thể thao mạo hiểm phải có đủ nhân viên chuyên môn sau đây:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Người hướng dẫn tập luyện thể thao đáp ứng một trong các điều kiện sau:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Là huấn luyện viên hoặc vận động viên phù hợp với hoạt động thể thao đăng ký kinh doanh có đẳng cấp từ cấp 2 trở lên hoặc tương đương;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Có bằng cấp về chuyên ngành thể dục, thể thao từ bậc trung cấp trở lên phù hợp với hoạt động thể thao đăng ký kinh doanh;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Được tập huấn chuyên môn thể thao theo quy định của Bộ trưởng Bộ Văn hóa, Thể thao và Du lịch.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Nhân viên y tế thường trực hoặc văn bản thỏa thuận với cơ sở y tế gần nhất về nhân viên y tế để sơ cứu, cấp cứu người tham gia hoạt động thể thao mạo hiểm trong trường hợp cần thiết.</span></span></div>
