<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Bước 1: Doanh nghiệp đề nghị cấp giấy chứng nhận đủ điều kiện kinh doanh hoạt động thể thao gửi hồ sơ đến Trung tâm Phục vụ hành chính công tỉnh.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Địa chỉ: 70 Lê Hồng Phong, thành phố Kon Tum.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Thời gian: Vào giờ hành chính từ thứ 2 đến thứ 6 hàng tuần.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Bước 2: Cán bộ tiếp nhận kiểm tra tính hợp lý và đầy đủ của hồ sơ:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Trường hợp hồ sơ đầy đủ thì công chức viết giấy hẹn </span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Trường hợp hồ sơ còn thiếu và không hợp lệ thì công chức hướng dẫn về chỉnh sửa, bổ sung.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Trường hợp hồ sơ cần sửa đổi, bổ sung Sở Văn hóa, Thể thao và Du lịch thông báo trực tiếp hoặc bằng văn bản những nội dung cần sửa đổi, bổ sung đến doanh nghiệp trong thời hạn 03 ngày làm việc, kể từ ngày nhận hồ sơ.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Trong thời hạn bảy ngày làm việc, kể từ ngày nhận đủ hồ sơ hợp lệ, Sở Văn hóa, Thể thao và Du lịch phải kiểm tra các điều kiện kinh doanh hoạt động thể thao của doanh nghiệp theo quy định và cấp giấy chứng nhận đủ điều kiện kinh doanh hoạt động thể thao.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Trường hợp không cấp, Sở Văn hóa, Thể thao và Du lịch trả lời bằng văn bản và nêu rõ lý do.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Bước 3: Trả kết quả tại Trung tâm Phục vụ hành chính công tỉnh hoặc qua đường bưu điện.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Thời gian: Vào giờ hành chính từ thứ 2 đến thứ 6 hàng tuần.</span></span></div>

<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Bản tóm tắt tình hình chuẩn bị các điều kiện kinh doanh hoạt động thể thao (theo mẫu). </span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Bản sao Giấy chứng nhận đăng ký doanh nghiệp; bản sao văn bằng, chứng chỉ, giấy chứng nhận của nhân viên chuyên môn đối với cơ sở kinh doanh hoạt động thể thao thuộc Danh mục hoạt động thể thao bắt buộc có hướng dẫn tập luyện hoặc cơ sở kinh doanh hoạt động thể thao mạo hiểm.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">b) Số lượng hồ sơ: 01 (bộ).</span></span></div>

<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(1) Có nguồn tài chính bảo đảm hoạt động kinh doanh hoạt động thể thao. Nguồn tài chính do cơ sở kinh doanh hoạt động thể thao tự chịu trách nhiệm</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(2) Cơ sở vật chất, trang thiết bị tập luyện </span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">a) Sàn tập có kích thước từ 08m x 08m trở lên.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">b) Mặt sàn bằng phẳng, không trơn trượt và được trải thảm hoặc đệm mềm.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">c) Khoảng cách từ sàn nhà đến trần ít nhất là 03m.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">d) Hệ thống âm thanh bảo đảm cường độ ít nhất từ 90dBA trở lên.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">đ) Ánh sáng bảo đảm từ 150 lux trở lên.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">e) Có túi sơ cứu theo quy định của Bộ Y tế.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">g) Trang bị các dụng cụ bổ trợ phục vụ người tập: Máy chạy bộ, tạ, bục, gậy, vòng, bóng.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">h) Có khu vực thay đồ và nhà vệ sinh.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">i) Có bảng nội quy bao gồm những nội dung chủ yếu sau: Đối tượng tham gia tập luyện, giờ tập luyện, trang phục tập luyện và các biện pháp bảo đảm an toàn khi tập luyện. </span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(3) Trang thiết bị tập luyện, thi đấu </span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">a) Thực hiện theo quy định tại các điểm b, c, đ, e và h mục (2) nêu trên. </span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">b) Sàn thi đấu có kích thước ít nhất là 12m x 12m. </span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">c) Hệ thống âm thanh bảo đảm cường độ ít nhất từ 120dBA trở lên.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(4) Mật độ tập luyện</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">a) Mật độ tập luyện trên sàn bảo đảm ít nhất 02m2 /01 người tập.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">b) Mỗi người hướng dẫn tập luyện hướng dẫn không quá 30 người trong một buổi tập.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(5) Nhân viên chuyên môn</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">a) Đối với cơ sở kinh doanh hoạt động thể thao thuộc một trong các trường hợp: </span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Cung cấp dịch vụ hướng dẫn tập luyện thể thao;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Kinh doanh hoạt động thể thao thuộc Danh mục hoạt động thể thao bắt buộc có hướng dẫn tập luyện. Danh mục hoạt động thể thao bắt buộc có hướng dẫn tập luyện do Bộ trưởng Bộ Văn hóa, Thể thao và Du lịch quy định.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Phải có người hướng dẫn tập luyện thể thao đáp ứng một trong các điều kiện sau:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Là huấn luyện viên hoặc vận động viên phù hợp với hoạt động thể thao đăng ký kinh doanh có đẳng cấp từ cấp 2 trở lên hoặc tương đương;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Có bằng cấp về chuyên ngành thể dục, thể thao từ bậc trung cấp trở lên phù hợp với hoạt động thể thao đăng ký kinh doanh;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Được tập huấn chuyên môn thể thao theo quy định của Bộ trưởng Bộ Văn hóa, Thể thao và Du lịch.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">b) Đối với cơ sở kinh doanh hoạt động thể thao mạo hiểm phải có đủ nhân viên chuyên môn sau đây:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Người hướng dẫn tập luyện thể thao đáp ứng một trong các điều kiện sau:</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Là huấn luyện viên hoặc vận động viên phù hợp với hoạt động thể thao đăng ký kinh doanh có đẳng cấp từ cấp 2 trở lên hoặc tương đương;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Có bằng cấp về chuyên ngành thể dục, thể thao từ bậc trung cấp trở lên phù hợp với hoạt động thể thao đăng ký kinh doanh;</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Được tập huấn chuyên môn thể thao theo quy định của Bộ trưởng Bộ Văn hóa, Thể thao và Du lịch.</span></span></div>
<div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Nhân viên y tế thường trực hoặc văn bản thỏa thuận với cơ sở y tế gần nhất về nhân viên y tế để sơ cứu, cấp cứu người tham gia hoạt động thể thao mạo hiểm trong trường hợp cần thiết.</span></span></div>
