<p><span style="font-size: medium;"><span style="font-family: "Times New Roman";"><strong>Thủ tục: Cấp giấy chứng nhận đủ điều kiện kinh doanh của doanh nghiệp kinh doanh hoạt động thể thao tổ chức hoạt động Khiêu vũ thể thao.</strong> </span></span></p> <table border="1" cellspacing="0" cellpadding="0" width="643"> <tbody> <tr> <td width="170" valign="top"> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Trình tự thực hiện:</span></span></div> </td> <td width="472"> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Bước 1: Doanh nghiệp đề nghị cấp giấy chứng nhận đủ điều kiện kinh doanh hoạt động thể thao gửi hồ sơ đến Trung tâm Phục vụ hành chính công tỉnh.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Địa chỉ: 70 Lê Hồng Phong, thành phố Kon Tum.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Thời gian: Vào giờ hành chính từ thứ 2 đến thứ 6 hàng tuần.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Bước 2: Cán bộ tiếp nhận kiểm tra tính hợp lý và đầy đủ của hồ sơ:</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Trường hợp hồ sơ đầy đủ thì công chức viết giấy hẹn </span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Trường hợp hồ sơ còn thiếu và không hợp lệ thì công chức hướng dẫn về chỉnh sửa, bổ sung.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Trường hợp hồ sơ cần sửa đổi, bổ sung Sở Văn hóa, Thể thao và Du lịch thông báo trực tiếp hoặc bằng văn bản những nội dung cần sửa đổi, bổ sung đến doanh nghiệp trong thời hạn 03 ngày làm việc, kể từ ngày nhận hồ sơ.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Trong thời hạn bảy ngày làm việc, kể từ ngày nhận đủ hồ sơ hợp lệ, Sở Văn hóa, Thể thao và Du lịch phải kiểm tra các điều kiện kinh doanh hoạt động thể thao của doanh nghiệp theo quy định và cấp giấy chứng nhận đủ điều kiện kinh doanh hoạt động thể thao.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Trường hợp không cấp, Sở Văn hóa, Thể thao và Du lịch trả lời bằng văn bản và nêu rõ lý do.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Bước 3: Trả kết quả tại Trung tâm Phục vụ hành chính công tỉnh hoặc qua đường bưu điện.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Thời gian: Vào giờ hành chính từ thứ 2 đến thứ 6 hàng tuần.</span></span></div> </td> </tr> <tr> <td width="170" valign="top"> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Cách thức thực hiện:</span></span></div> </td> <td width="472"> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Trực tiếp tại trụ sở Trung tâm Phục vụ hành chính công tỉnh hoặc qua đường bưu điện (hoặc trực tuyến nếu có). </span></span></div> </td> </tr> <tr> <td width="170" valign="top"> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Thành phần, số lượng hồ sơ:</span></span></div> </td> <td width="472"> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">a) Thành phần hồ sơ: </span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Đơn đề nghị cấp giấy chứng nhận đủ điều kiện kinh doanh hoạt động thể thao (theo mẫu).</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Bản tóm tắt tình hình chuẩn bị các điều kiện kinh doanh hoạt động thể thao (theo mẫu). </span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Bản sao Giấy chứng nhận đăng ký doanh nghiệp; Nguồn tài chính do cơ sở kinh doanh hoạt động thể thao tự chịu trách nhiệm</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(2) Cơ sở vật chất, trang thiết bị tập luyện</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">a) Sàn tập luyện có kích thước ít nhất 08 m x 11m. </span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">b) Mặt sàn bằng phẳng, không trơn trượt, bề mặt sàn được lát bằng một trong các vật liệu sau: Gỗ tự nhiên, gỗ công nghiệp, gạch men, đá hoa. </span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">c) Khoảng cách từ sàn đến điểm thấp nhất trên trần hoặc mái che ít nhất là 2,8m.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">d). Hệ thống âm thanh có cường ít nhất từ 90dBA trở lên.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">đ) Ánh sáng phục vụ tập luyện ít nhất từ 150Lux trở lên.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">e) Có ghế ngồi và gương.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">g) Có túi sơ cứu theo quy định của Bộ Y tế.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">h) Có khu vực thay đồ và nhà vệ sinh.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">i) Có bảng nội quy bao gồm những nội dung chủ yếu sau: Đối tượng tham gia tập luyện, trang phục tập luyện, giờ tập luyện, biện pháp đảm bảo an toàn khi tập luyện.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(3) Cơ sở vật trang thiết bị tập luyện, thi đấu </span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">a) Thực hiện theo quy định tại các khoản a, b, c, d, đ, e, g và h mục (2) nêu trên.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">b) Ánh sáng phục vụ thi đấu ít nhất từ 200 Lux trở lên.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(4) Cơ sở vật chất, trang thiết bị biểu diễn</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">a) Thực hiện theo quy định tại các điểm c, d, h mục (2) và điểm b mục (3) nêu trên. </span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">b) Mặt sàn biểu diễn bằng phẳng, không trơn trượt.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">c) Trang phục không được làm bằng chất liệu hoặc màu trong suốt và phải che kín những vùng nhạy cảm của cơ thể người biểu diễn.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(5) Mật độ tập luyện</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">a) Mật độ tập luyện trên sàn bảo đảm ít nhất 02m2 /01 người tập.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">b) Mỗi người hướng dẫn tập luyện hướng dẫn không quá 35 người trong một buổi tập.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">(6) Nhân viên chuyên môn</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">a) Đối với cơ sở kinh doanh hoạt động thể thao thuộc một trong các trường hợp: </span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Cung cấp dịch vụ hướng dẫn tập luyện thể thao;</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Kinh doanh hoạt động thể thao thuộc Danh mục hoạt động thể thao bắt buộc có hướng dẫn tập luyện. Danh mục hoạt động thể thao bắt buộc có hướng dẫn tập luyện do Bộ trưởng Bộ Văn hóa, Thể thao và Du lịch quy định.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Phải có người hướng dẫn tập luyện thể thao đáp ứng một trong các điều kiện sau:</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Là huấn luyện viên hoặc vận động viên phù hợp với hoạt động thể thao đăng ký kinh doanh có đẳng cấp từ cấp 2 trở lên hoặc tương đương;</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Có bằng cấp về chuyên ngành thể dục, thể thao từ bậc trung cấp trở lên phù hợp với hoạt động thể thao đăng ký kinh doanh;</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Được tập huấn chuyên môn thể thao theo quy định của Bộ trưởng Bộ Văn hóa, Thể thao và Du lịch.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">b) Đối với cơ sở kinh doanh hoạt động thể thao mạo hiểm phải có đủ nhân viên chuyên môn sau đây:</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Người hướng dẫn tập luyện thể thao đáp ứng một trong các điều kiện sau:</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Là huấn luyện viên hoặc vận động viên phù hợp với hoạt động thể thao đăng ký kinh doanh có đẳng cấp từ cấp 2 trở lên hoặc tương đương;</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Có bằng cấp về chuyên ngành thể dục, thể thao từ bậc trung cấp trở lên phù hợp với hoạt động thể thao đăng ký kinh doanh;</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">+ Được tập huấn chuyên môn thể thao theo quy định của Bộ trưởng Bộ Văn hóa, Thể thao và Du lịch.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Nhân viên y tế thường trực hoặc văn bản thỏa thuận với cơ sở y tế gần nhất về nhân viên y tế để sơ cứu, cấp cứu người tham gia hoạt động thể thao mạo hiểm trong trường hợp cần thiết.</span></span></div> </td> </tr> <tr> <td width="170" valign="top"> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Căn cứ pháp lý:</span></span></div> </td> <td width="472"> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Luật thể dục, thể thao số 77/2006/QH11 ngày 29/11/2006. </span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Luật sửa đổi, bổ sung một số điều của Luật TDTT số 26/2018/QH14 ngày 14/6/2018.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Nghị định số 112/2007/NĐ-CP ngày 26/6/2007 của Chính phủ.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Nghị định số 106/2016/NĐ-CP ngày 01/7/2016 của Chính phủ.</span></span></div> <div><span style="font-size: medium;"><span style="font-family: "Times New Roman";">- Thông tư số 07/2018/TT-BVHTTDL ngày 30/01/2018 của Bộ trưởng Bộ Văn hóa, Thể thao và Du lịch.</span></span></div> </td> </tr> </tbody> </table> <p><a href="/LinkClick.aspx?fileticket=KUiDxt1skFM%3d&tabid=403"><span style="font-size: medium;"><span style="font-family: "Times New Roman";">Tải mẫu đơn</span></span></a></p>
